All players and cheerleaders MUST have all six (6) forms completed (NPJT Family info, Football/cheer application, medical/liability, photo release, social media policy and Covid liability waiver). New and recent player/cheer require updated photo and copy of a birth certificate.
Registration cost is $135 per player/cheerleader. Check or money order payable to NPJT. EXACT Cash is also accepted. Child discount as follows: 1st child $135, 2nd child $135, 3rd child $110, 4th child $85, each additional child $85. (Sorry – we do not have Venmo or other online payment options)
Refunds will only be issued until the last day of the second week of practice at the practice field during practice hours.
